The Case Manager assesses new referrals, coordinates services with clinical and medical teams, and monitors patient cases by evaluating treatment response and intervening in crises. Responsibilities also include communicating progress weekly and assisting with discharge plans and aftercare referrals.
Candidates must possess a Bachelor’s degree in the Human Service field from an accredited college or university and hold a valid/current Driver's License. The role may also require the ability to travel up to 15% of the time.
