The nurse coordinates a broad range of health and social services within a Naturally Occurring Retirement Community (NORC) or Senior Living Community to support older residents aging in place. Responsibilities include facilitating resident engagement in self-care, providing health outreach, conducting assessments, making referrals, and acting as a health care advocate during transitions.
Candidates must possess a New York State Registered Professional Nurse license, current CPR/BLS certification, and a minimum of two years of experience as an RN in a Licensed Home Care Service Agency (LHCSA) or Certified Home Health Agency (CHHA). An Associate's Degree in nursing is required, while a Bachelor's Degree is preferred, along with prior experience working with aging populations and conducting clinical health needs assessments.
VNS Health Community Health RN's serve as a member of an interdisciplinary care team within a NORC (Naturally Occurring Retirement Community) or senior living community, coordinating a broad range of health and social services to support older adults in aging safely in place. NORCs achieve this by facilitating and integrating existing community-based health and social services, while identifying and organizing additional supports needed to help residents remain independent and engaged in their homes and communities.
