About the role

The Social Worker collaborates with the healthcare team, patient, and family to coordinate timely psychosocial assessments and develop patient-centered discharge plans. Key duties include providing age-specific care, implementing comprehensive plans based on identified needs, and linking patients to necessary community resources.

A Brief Overview Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. What You Will Do Provides psychosocial assessment / intervention to assist patients/families.

Provides age specific care to patients/families in all age groups. Develops and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs.

Develops plans with attention to individual patient and family goalsProvides linkage to community resources to facilitate optimal care Communicates/collaborates with the work team using a multidisciplinary approach Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.

Education Master's Degree from an accredited school of Social Work (Required) Work Experience Hospital/Health Agency experience. (Preferred) Knowledge, Skills, & Abilities Computer skills (Required proficiency) Theoretical and clinical knowledge regarding health care delivery (Required proficiency) Multi-tasks and prioritizes work (Required proficiency) Communicates effectively with persons of various backgrounds (Preferred proficiency) Licenses and Certifications Licensed Social Worker (LSW) (Required) Preferred Qualifications and Miscellaneous Information: At least 1 year of recent experience is strongly preferred.

Previous work in psychiatry or mental health is also strongly preferred.

This is a locked, in-patient psychiatric unit.

Responsibilities include performing assessments, counseling with the patients and families, providing referrals and community resources, discharge planning, scheduling and planning outpatient follow-up.

This position will cover for PTO’s and government requirements.

It is needed for rotating weekends and some weekdays as needed.

Requirements

A Master's Degree from an accredited School of Social Work is required, along with a Licensed Social Worker (LSW) certification. Preferred qualifications include at least one year of recent experience, especially in psychiatry or mental health within a hospital or health agency setting.
