The RN Case Manager administers skilled nursing care and coordinates care with the interdisciplinary team, patients, and families. They are responsible for developing and revising care plans to ensure quality and continuity of care.

Job Qualifications: Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Licensure: Current State license as a Registered Nurse, current Driver’s License.

Candidates must be graduates of an accredited nursing program and hold a current RN license. One year of clinical experience is required, with home health experience preferred.
