Physical therapists perform comprehensive physical and functional patient assessments, develop individualized plans of care with defined goals, and provide evidence-based direct intervention to patients in their homes. They are also responsible for conducting home safety evaluations and educating patients and families on home programs and available resources.
Candidates must possess an active State Physical Therapist License and current CPR with BLS certification, along with a valid driver's license and appropriate auto insurance for travel. Essential qualifications include strong organizational skills, the ability to work with minimal supervision, computer literacy for EMR documentation, and the capacity to remain calm during emergencies.

Active State Physical Therapist License Current CPR with BLS card Valid driver’s license, at least state minimum auto insurance, and an operational vehicle Possess or obtain a good understanding of Federal, State, and local laws and regulatory guidelines governing home health operations Great organization skills and an ability to work with minimal supervision Capacity to appropriately supervise others Ability to remain calm in emergency or crisis situations Computer/Technology literacy and experience documenting on an Electronic Medical Record (EMR) system
