The Nurse Care Manager delivers medication services, triage, and case management within the health center and medical respite program, actively participating in the treatment team to ensure individualized, patient-centered care. Essential duties include building patient relationships, performing medical triage, processing program referrals, and educating staff and patients on program expectations.
Candidates must possess either a BSN with a minimum of 1 year of behavioral health care experience or an RN license with 2 years of behavioral health care experience. Required qualifications also include a valid PA Registered Nurse License and successful completion of Act 33/34/73 Clearances and pre-employment drug testing.
Medical Respite provides short-term residential care for individuals experiencing homelessness who are too medically vulnerable to safely recover on the streets or in a shelter environment, yet do not require hospitalization. The Medical Respite RN Nurse Care Manager plays a vital role in supporting recovery and promoting stability through clinical care and monitoring, medication management, care coordination, patient education, and discharge planning. This position works collaboratively with interdisciplinary teams and community partners to ensure compassionate, patient-centered care for a highly vulnerable population. Essential
Build trusting relationships with patients through high-touch engagement. Perform medical triage in collaboration with healthcare staff. Accept and process referrals for the medical respite program. Educate patients and hospital staff about the Medical Respite program and participation expectations. Conduct medication reconciliation and provide medication education in partnership with providers and pharmacy staff. Develop and facilitate chronic disease management and healthy lifestyle programs using evidence-based practices. Coordinate internal and external care, including specialty care, tobacco cessation, housing, transportation, and health benefits. Identify and address barriers to improved health outcomes for medically complex patients. Implement systematic strategies for assessments, enrollment, care planning, documentation, and value measurement for patient care. Participate in a multidisciplinary team to integrate primary and behavioral healthcare services. Offer ongoing consultation with treatment teams, hospitals, specialty providers, pharmacies, insurance companies, patients, and families. Provide crisis intervention as needed. Administer medication injections as required. Assist patients with daily living skills and activities. Support patients in preparing for medical appointments. Encourage and facilitate patient engagement in their community. Enhance programming by networking and collaborating with internal and external partners.
