The role involves providing psychosocial assessment, counseling, crisis intervention, and information/referral services on a consultation basis for hospice patients and families. This includes employing counseling skills to aid understanding and acceptance of care recommendations and intervening in crisis situations.
The minimum qualifications require a Master’s Degree in Social Work and one year of experience as a social worker or applicable mental health experience. Candidates must also possess knowledge of patient rights, relevant laws like HIPAA, psychological principles, high emotional intelligence, and strong problem-solving skills.
Master’s Degree in Social Work 1 year experience working as a social worker or applicable mental health experience. Ability to work effectively with people from a broad range of backgrounds. Ability to adhere to the National Association of Social Workers Code of Ethics. Knowledge of patient rights and laws relative to those rights, such as HIPAA. Knowledge of psychological principles and theories. Must possess a high degree of emotional intelligence, and empathy towards others. Skill in using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems. Preferred Qualifications LSW - Licensed Social Worker - State of Pennsylvania Physical Demands Lift and carry 7 lbs., continuous sitting >67%, frequent keyboard use/repetitive motion, frequent fine motor activity/wrist position deviation.
