Case Managers facilitate direct patient care and skilled nursing visits, performing tasks such as medication management, wound care, vital sign monitoring, and patient education according to the established plan of care. Duties also include completing patient care tasks, following and reevaluating the plan of care, working scheduled shifts, and completing all required clinical nurse notes and skilled care services.
Candidates must hold a current license as a Licensed Practical Nurse or Registered Nurse from the Virginia Board of Nursing and must possess experience working in a home care agency with direct patient care experience. Dedication and dependability are also required qualities for this role.
Case Managers are a critical part of the agency and must have experience working in the home health setting. Case Managers facilitate direct patient care and skilled nursing visits. Case Managers perform tasks such as medication management, wound care, vital sign monitoring, and patient education in accordance with the established plan of care.
