The Physical Therapist will provide care under the direction of the Director of Clinical Services, including taking initial patient history, making evaluations, and consulting with physicians regarding treatment changes. Responsibilities also involve directing and supervising assistants/aides, instructing patients/families in home programs, and completing required documentation like OASIS assessments and utilizing ICD-10 codes.

Summary of Responsibilities

Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services. Work in conjunction with all other home care personnel in patient care planning. Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned. Take initial patient history and make initial evaluation. Consult with physicians regarding change in treatment. Instruct patients and family in home programs and activities of daily living. Participates in Quality Assessment and Performance Improvement activities as assigned. Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition. Appropriately utilizes ICD-10 codes.

Requirements

Graduate from a physical therapist curriculum approved by the American Physical Therapy Association. Currently licensed as a physical therapist in the state of Washington. Two (2) years' experience as a physical therapist, preferred. Home Health experience, preferred. Detail oriented, self motivated and capable of efficient time management. Critical thinker capable of working collaboratively with a team. Valid WA State driver's license.
