This role involves determining patient needs via telephonic or teleportal contact to enhance daily living and assisting patients in identifying and meeting established goals through an individual plan of care. The social worker will also coordinate community resources, maintain electronic health records, and facilitate transitions from inpatient care to home settings.
Candidates must possess a completed Bachelor's degree in psychology, sociology, or social work, and must hold a valid State License or certification to practice if applicable. Experience in the medical or psychiatric field is required, along with a valid Driver's License.
