The Case Manager will provide a complete continuum of quality care by interacting with members via phone, virtual, or in-person methods, assisting them with the transition from incarceration back to the community through assessment and care planning. Responsibilities also include partnering with the care team to reduce emergency department utilization, hospital readmissions, and recidivism.
Candidates must possess an independently active and unrestricted license in Kentucky, either as a clinician with a Master's degree in a behavioral health discipline or an RN with a BSN and 3+ years of behavioral health experience. A minimum of two years of post-licensure experience in a related mental health environment and one year working with justice-involved populations are required.
Independently licensed clinician, with a Master's degree in Psychology, Social Work, Counseling, or related behavioral health discipline, OR an RN (BSN required) with 3+ years of experience in behavioral health Licenses must be independent, active, and unrestricted in the state of Kentucky 2+ years of post-licensure experience in a related mental health environment 1+ years of direct experience with justice-involved populations Proficient computer skills and good working knowledge of Microsoft Word and Outlook Access to high-speed internet and a dedicated, distraction-free workspace at home Reliable transportation and the ability to travel within designated service delivery area Permanent residence in the state of Kentucky
