As an RN this team, you will collaborate with physicians and fellow caregivers to support patients within our Care Center.

A caregiver in this role will work 7:00pm to 7:30am.
Caregivers will also work every other weekend and 2 holidays a year.
A caregiver who excels in this role will: Provide supervision, assessment, planning and direct patient care to hospice patients.
Perform accurate and complete admissions assessments and re-assessments.
Assess patients for safety risks, abuse and neglect.
Assess, document and manage patient’s pain.
Prioritize and document patient problems.
Administer medications and treatments safely and accurately with a focus on patient comfort.
Coordinate care for managing a caseload of patients.
