The Case Manager supports young adults (ages 18-25) in securing and maintaining stable housing while developing life skills through flexible one-on-one guidance and resource connection. Key duties include conducting intake assessments, developing individualized housing plans, coordinating various supportive services, and monitoring client vital signs.
A Bachelor's degree in a related field is preferred, along with 1-2 years of case management experience working with populations experiencing homelessness, mental illness, or trauma. Candidates must possess knowledge of Housing First principles and be able to pass background checks and handle physical demands like lifting up to 50 pounds.
The Supportive Housing Case Manager - CNA (Ages 18–25) supports young adults in securing and keeping stable housing while building life skills and independence. Using a positive, youth-centered approach, you’ll carry a small caseload (12–18 clients), offer flexible one-on-one guidance, and connect them to education, jobs, and community resources. This is a rewarding role for anyone who enjoys cheering young people on as they step confidently into adulthood.
