This position proactively coordinates care activities for patients, working closely with the Integrated Care Team to achieve quality and cost-effective health outcomes. The primary focus is assisting detention center residents who are transitioning back into the community.
Candidates must self-identify as having lived experience with addiction/recovery and criminal justice involvement, including release from jail at least two years prior. A High School diploma or GED plus a minimum of one year of experience delivering peer support services is required.
