The Care Coordinator manages and facilitates comprehensive care plans for individuals with intellectual and developmental disabilities, ensuring they receive coordinated, person-centered services to promote health and independence. Core functions include assessing needs, developing support plans, coordinating services, and acting as the enrollee's advocate.
require a Bachelor's degree in a relevant social services field or RN license, along with a minimum of two years of experience working with individuals with intellectual developmental disabilities, though LPNs require four years of experience. Preferred qualifications include a Master's degree or certification in care coordination.
