As a Home Health RN, the individual will act as a clinical leader, delivering personalized nursing care, and developing, implementing, and adjusting comprehensive care plans to promote patient independence. This role also involves collaborating with a multidisciplinary team, educating patients and families, and overseeing LPNs, LVNs, and Home Health Aides.
Candidates must be graduates of an accredited nursing program holding an active RN license in the service state, with a minimum of one year of nursing experience, though Home Health experience is preferred. Essential requirements include valid driver's license, reliable transportation, current CPR/BLS certification, and strong critical thinking and case management abilities.
