• SUMMARY: The Licensed Practical Nurse (LPN) – Integrated Medical Care Coordinator serves as a key clinical member of the integrated care team within a Certified Community Behavioral Health Center (CCBHC). This is a senior-level LPN role requiring independent clinical judgment, strong organizational skills, and proactive care coordination within the LPN scope of practice. The LPN Medical Care Coordinator supports individuals with complex medical and behavioral health needs by conducting physical health screenings, managing high-risk medical caseloads, coordinating referrals, monitoring medication adherence, and ensuring continuity of care. This role requires a seasoned and dependable nurse who takes initiative, works systematically, and follows through consistently on client care needs.
• QualificationsMINIMUM
• QUALIFICATIONS: Graduate of an accredited Practical Nursing program Active and unrestricted LPN license (Pennsylvania or Compact State accepted) Minimum 3–5 years of licensed clinical nursing experience required Prior experience in behavioral health, substance use treatment, community health, or care coordination strongly preferred Demonstrated ability to work independently within LPN scope of practice Proven history of reliability, professional accountability, and consistent attendance Strong clinical judgment and ability to manage responsibilities with minimal supervision Experience managing a caseload and tracking client progress Strong documentation and organizational skills Only applicants meeting licensure and experience requirements will be considered.
