The Medical Care Coordinator identifies client medical needs via screenings and records to create individualized physical health plans, offering support and referrals to external providers. They also provide disease management education, identify barriers to care, and advocate for clients navigating healthcare systems.
Candidates must be a graduate of a Board-Certified LPN Program and hold current, active PA LPN Licensure. A minimum of one year of LPN experience is required, along with knowledge of mental health and substance abuse systems and the ability to work effectively with individuals facing serious mental illness or substance abuse issues.
