Assesses service needs and links individuals and families to health and social service systems to address barriers to substance use treatment and recovery. Coordinates with referral sources and maintains accurate client case records and progress reports.
Requires an associate degree in social work, psychology, criminal justice, or a related human services field, or an equivalent combination of experience. Must be able to obtain PA state and FBI clearances and possess a valid insured vehicle for transport.
