About the role

The Registered Nurse Case Manager is responsible for providing end-of-life care and Hospice services in an Inpatient setting, focusing on holistic, family-centered care to anticipate and treat symptoms. Key duties include implementing care plans for comfortable dying, acting as a family educator/advocate, completing clinical documentation, and coordinating patient transitions.

Hospice GIP Registered Nurse (RN) Case Manager - Beaumont Hospice SCHEDULE: Four 10-hour shifts or five 8-hour shifts with one weekend per month rotation.

One winter and summer holiday AGENCY: Beaumont Hospice LOCATION: We are currently servicing 4 Corewell hospital locations (Troy, Royal Oak, Dearborn, Trenton) and looking for a float RN to assist daily where needed.

Two shift options: HOW YOU'LL MAKE A DIFFERENCE: Care and compassion are at the heart of what you'll do as a Hospice, General InPatient Nurse-Case Manager.

You'll help deliver care in a holistic, family-centered manner anticipating, preventing and treating undesirable symptoms or secondary symptoms.

HOW YOU'LL WORK: You'll be responsible for providing end-of-life care and Hospice services to patients in the Health System Inpatient Hospice setting in a manner consistent with standards of practice. You will assist in determining the need to change the level of care based upon assessment and collaboration with the Attending Physician or Medical Director. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure and effective grieving for patient and family. Family Educator/Advocate: Maintain the dignity of the dying patient while supporting their unique spiritual and cultural beliefs, provide appropriate support at the time of death, including disposition of medications, notification of funeral home, etc. according to agency policy.

Policies

Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.

Communication

Provide referral sources with information on hospice programs and the coordination of patient care for optimal transition from the hospital to discharge destination in a hospice care setting wherever the patient calls home.

Operations

Attend interdisciplinary team meetings for full team discussions, consultation and evaluation of patient/family needs.

SKILLS: Compassionate communicator with a positive attitude. Patience is a virtue when working with patients, families, physicians, and coworkers. Independent worker with demonstrated skills in patient care and education. Attention to detail is critical, as is being observant and following directions.

Requirements

Bachelor's Degree in Nursing preferred or Graduate of Diploma or Associate Degree program in Nursing within the state of employment. Minimum 3 years inpatient clinical experience, preferably hospice.
