As a Primary Care Physician, you will operate within a team-based model, collaborating with primary and specialty care providers, hospital partners, and skilled nursing facilities to deliver high-quality, evidence-based medicine to a diverse patient population. Responsibilities include focusing on value-based care, engaging in shared decision-making, and working alongside integrated behavioral health providers.
