Position overview

Queens Community House is seeking a highly skilled Senior Health Monitor to join our Queens Childcare Network team. In this role, you will be responsible for monitoring and assessing client health, ensuring high-quality care, and supporting the development and implementation of individualized care plans.

The Senior Health Monitor plays a critical role in improving client outcomes and enhancing the overall well-being of children and families served across our network.

  • Key responsibilities
  • Conduct comprehensive and ongoing health assessments of clients
  • Document findings accurately and maintain up-to-date health records
  • Collaborate with healthcare professionals to develop and implement care plans
  • Monitor and report changes in client health status in a timely manner
  • Provide education, guidance, and support to clients and their families
  • Ensure compliance with all health, safety, and regulatory requirements

Minimum Qualifications

  • Bachelor’s degree in Nursing or a related field
  • Minimum of 5 years of experience in a healthcare setting
