This role involves planning, organizing, and implementing social work, discharge planning, and case management services for residents and their families within a healthcare system. The worker will also lead efforts in addressing resident financial matters in collaboration with management and regulatory bodies, functioning as the Qualified Social Worker for skilled nursing facilities.
A Bachelor's degree in Social Work is required for this position, which demands the ability to handle pressure, meet deadlines, maintain accuracy, and manage stressful situations and conflicts with diverse groups of people. Adherence to all organizational policies and licensing board regulations is mandatory.
