The Dietician will be responsible for delivering nutritional counseling to all patients, assessing their needs, and developing individualized nutrition care plans and education in collaboration with healthcare personnel. Duties include creating dietary recommendations for chronic conditions and instructing patients and families on diet modifications and supplement use.
Candidates must be currently registered with the Commission on Dietetic Registration (CDR) and hold a valid license to practice dietetics in Texas, along with current BLS/CPR certification. A thorough understanding of medical nutrition therapy and a special interest in community medicine are required, with bilingual proficiency in English and Spanish being preferred.
Under the direction of the Executive Vice President Medical, the Dietician will deliver nutritional counseling to all Avenue 360 patients. They will deliver care appropriate for an outpatient, primary care FQHC with a multicultural, multilingual patient population. What you'll do
· Assess patients’ nutritional needs and current health plans to help develop and implement nutrition care plans and education. · Work with physicians and health care personnel to assess health, environmental, and cultural factors, which may influence the nutritional needs and education of patients or clients. · Create individualized dietary recommendations for individuals and groups for the management of chronic health conditions such as diabetes, wasting syndrome, hypertension, obesity, etc. · Educate patients and their families on nutritional principles, dietary plans, diet modifications, and healthy food selection and preparation as indicated by diagnosis and medical history. · Instruct patients and their families on the use of nutritional supplements, vitamins, and weight gain products for the management of chronic disease states. · Coordinate nutrition counseling services. · Plan and conduct training programs in dietetics, nutrition, and institutional management and administration for staff, health-care personnel, and the general public.
Education, Licensure/ Certification: · Must currently be registered with the Commission on Dietetic Registration (CDR) and must hold a valid license to practice dietetics in the State of Texas. · BLS/CPR required.
· Have a thorough understanding of the principles and practices of medical nutrition therapy, medical management of chronic health conditions, and allied services. · Demonstrate a special interest in the area of community medicine. · Ability to relate to and educate patients and clients from culturally diverse communities/backgrounds. · Bilingual in English and Spanish preferred.
· Participates in trainings required by the agency, funding source(s), and/or as required by licensure if applicable.
