Provide home-based care to patients with various conditions under the supervision of an RN, collaborating with a team to deliver quality care. Responsibilities include taking vitals, performing assessments, documenting progress, assisting the RN, and educating patients and families on the care plan and safety.
Applicants must possess an active, unrestricted Licensed Practical Nurse (LPN) license in the State of Michigan and have at least one year of clinical experience as an LPN. Candidates also need current CPR Certification and reliable transportation with a valid driver's license and insurance.
