About the role

The Nurse Care Manager will assist with the implementation and integration of Healthcare Home programs into daily clinical operations, including monitoring critical chronic disease indicators using Population Health reports. Duties also involve identifying high-risk consumers, making referrals, and ensuring accurate record keeping for the assigned caseload.

Job Purpose

The LPN or RN Nurse Care Manager supports individuals in the population health-based program. In this role, you will help people manage their overall health, including chronic conditions, by providing screenings, education, and care coordination. You will work closely with a team to make sure each person receives the right care at the right time.

Qualifications

  • High school diploma or equivalent.
  • Current Missouri or compact state license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
  • Ability to meet clinical privileging requirements at FCC Behavioral Health.
  • Basic computer skills and ability to learn electronic health systems.
  • Core Job Duties
  • Perform basic annual health screenings (such as blood pressure, weight, and health history updates).
  • Complete finger stick tests to check important metabolic health markers (such as blood glucose and cholesterol).
  • Identify individuals who may be at higher health risk and help connect them to needed services.
  • Track and review health data using electronic care management systems.
  • Support people in understanding and improving their health through education and wellness activities.
  • Assist with group health programs for clients, staff, and the community.
  • Keep accurate, up-to-date health records.
  • Follow best practices and care guidelines.
  • Adherence to all applicable evidence-based practice models.
  • Orally communicate information effectively and accurately.
  • Assure program operates within the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.
  • Abide by program and agency policies and procedures.
  • Other job duties and special projects as assigned.

Requirements

Candidates must possess a High School Diploma or equivalent and be a Registered Nurse or Licensed Practical Nurse with a current Missouri license. Preferred qualifications include five years of primary care experience in Nursing and familiarity with healthcare technology.
