The Home Based Therapist provides clinical services to children and families to stabilize and strengthen their functioning. Responsibilities include maintaining clinical charts, providing treatment, and facilitating access to other services.
A Master’s degree in psychology, social work, or a related field is required, along with current licensure in Michigan and at least one year of experience with minors. Experience with multiple human service delivery systems is preferred.
Maintain clinical charts in an accurate and timely manner to include, but not limited to assessments, treatment plans, and all other MDCH chart requirements. Maintain a caseload size and weekly productivity expectations as determined by supervisor. Provide individual, group, and family treatment for children with mental health and co-occurring disorders (developmental disabilities and/or substance abuse). Provide education, consultation, and case management services. Facilitate client access to other service delivery systems, e.g., D.S.S. special education, etc. Assure the coordination and integration of foregoing services with those provided by Starfish Family Services. Make intra-agency referrals for evaluations, consultation, and other services, e.g., parent education. Maintain cooperative working relationships with educational, legal, health, social welfare, and mental health systems. Secure relevant reports from and provides reports to the foregoing system. Participate in the planning, development, and evaluation of agency programs. Attend mandatory agency meetings, trainings and supervisions. Participate in professional development activities, including in-service training, peer review, external workshops, and seminars, etc. May participate in special projects, e.g., research, community education, etc. Cooperate in any investigation related to personnel, licensing, accreditation or other circumstances. Work cooperatively as a member of a team with program staff and community resources. Accept and emulate the mission of SFS. Must maintain professional and ethical standards at all times. Assist in finding and directing clients in case of an emergency such as a fire or tornado. Adhere to all Lifespan policies and procedures as it relates to Documentation, Training requirements, CMH guidelines, and Confidentiality. COMPETENCIES AND ABILITIES: Knowledge of theory and practice of individual, group, and family systems treatment. Competence in the delivery of crisis intervention services, and in brief and time-limited therapy. Development and Implementation of Individualized Plan of Service utilizing Person Centered Planning. Competency in performing, or willingness to learn, a wide range of treatment modalities that are individualized and person centered. Competence in the delivery of crisis intervention services, and in brief and time-limited therapy. Competency, or willingness to learn, co-occurring treatment of mental health, substance abuse and/or developmental disabilities. Must be computer literate. Ability to make critical decisions independent of immediate supervision. Knowledge of community resources and linkages. Ability to effectively, appropriately, and accurately communicate both orally and in writing. Ability to maintain cooperative and effective working relationships with other staff members. Ability and willingness to abide by all SFS confidentiality policies. Ability to give and receive constructive feedback. Ability, desire, and motivation to work cooperatively with the program leadership team, as well as colleagues and all staff. Cultural sensitivity and competence in working with multi-cultural client populations. Must have reliable transportation in order to be able to meet with clients outside of the clinic setting. Ability to maintain professional and ethical standards. Ability to work with all members of the community regardless of race, gender, age and cultural or ethnic backgrounds. Ability to work flexible hours as needed (i.e., some evenings or weekend work is required). Ability to represent the agency in a professional manner. Ability to meet productivity expectation as assigned by program supervisor. LICENSING AND OTHER REQUIREMENTS: Must possess a current valid State of Michigan chauffeur’s driver’s license and automobile insurance that is insurable.
