Job Description: Summary

We are seeking a Speech and Language Pathologist (PRN) to join our Home Health Department in Port Charlotte. In this role, there is a real impact in the comfort of patients’ homes. As part of our compassionate, high‑performing care team, you’ll help individuals improve communication, swallowing safety, and overall quality of life through personalized, evidence‑based therapy.

‎

What You'll Do • Patient Assessment

Conduct comprehensive evaluations of patients' speech, language, cognitive-communication, and swallowing abilities to identify strengths, weaknesses, and treatment needs.

Treatment

Planning: Develop individualized treatment plans that address specific patient goals, including speech sound production, language development, fluency, voice disorders, and swallowing difficulties.

Therapeutic

Interventions: Provide direct therapy services, such as articulation therapy, language therapy, fluency therapy, voice therapy, and swallowing therapy, using evidence-based practices.

Patient

Education: Educate patients and their families on strategies to improve communication skills, swallowing safety, and overall quality of life.

Collaboration: Collaborate with other healthcare professionals, such as physicians, occupational therapists, and physical therapists, to develop comprehensive care plans and ensure optimal patient outcomes.

Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.

Compliance: Adhere to all relevant regulatory standards and guidelines, including HIPAA and Medicare regulations.

What We're Looking For • Licensure

Current and valid Speech-Language Pathologist (SLP) license in the state of Florida.

Experience: Minimum of 1 year of experience in hospital, rehab, or skilled nursing facility is required.

Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.

Professionalism: Strong work ethic, reliability, and a commitment to providing high-quality care.

Transportation: Reliable transportation to visit patients in their homes.

Compensation Range

$73,343.00

to

$110,014.00

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.

Requirements

A current and valid Speech-Language Pathologist license in Florida is required, along with a minimum of 1 year of experience in a relevant healthcare setting. Strong clinical skills and a commitment to high-quality patient care are essential.
