The Discharge Planner initiates discharge planning, coordinates aftercare appointments, and implements discharge plans based on the patient's level of care. This role also involves maintaining communication with teammates and community resources while ensuring accountability for services provided.
Candidates must possess a Bachelor’s Degree from an accredited institution in a related field, though applicable experience may substitute for the degree. A minimum of 3 to 5 years of clinical case management or discharge planning experience is required.

As a Discharge Planner, you will be responsible for initiating discharge planning for patients, working closely with the treatment team to schedule appropriate after care appointments and communicating with referral sources. You will coordinate, conduct, and implement discharge planning for patients at the levels of care being provided upon discharge. This role also works as a part of the treatment team and maintains effective communication with teammates and community resources and accountability for services provided.
