As a Primary Social Worker, you'll be an essential member of Unity's interdisciplinary hospice team, helping patients and families navigate the emotional, social, and practical aspects of end-of-life care.
You'll: Complete psychosocial assessments and develop individualized care plans.
Provide counseling, emotional support, education, and advocacy for patients and families.
Connect patients with community resources and coordinate referrals.
Participate in interdisciplinary care planning and collaborate with nurses, physicians, chaplains, aides, and volunteers.
Support bereavement planning and assist with funeral planning when requested.
Maintain timely and accurate documentation.
Uphold the Social Work Code of Ethics while delivering compassionate, patient-centered care.
Participate in a shared on-call rotation.

Bachelor’s in Social Work (BSW) from an institution accredited by the Council on Social Work Education; or bachelor’s degree in psychology, sociology, or other fields related to social work along with one year in a healthcare setting. Master’s of Social Work preferred. Certified or licensed as a social worker (CSW, CAPSW, CISW, or LCSW) by the State of Wisconsin required. Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage.
