The Physical Therapist will provide person-centered care by consulting with patients to assess conditions, developing individualized treatment plans, and administering prescribed physical therapy treatments to relieve pain and restore function. Responsibilities also include monitoring patient response, collaborating with the interdisciplinary team, and ensuring thorough documentation compliance.
Candidates must possess an active and valid Texas Physical Therapist license and be in good standing with all regulatory agencies. Experience in a Skilled Nursing Facility or long-term care setting is preferred, along with current knowledge of physical therapy practices.
