About the role

The Physical Therapist will provide person-centered care by consulting with patients to assess conditions, developing individualized treatment plans, and administering prescribed physical therapy treatments to relieve pain and restore function. Join our team as a full-Time physical therapist (Pt)

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our in-house team of outstanding professionals. In this role, you will provide person-centered care to help residents achieve positive outcomes and improve their mobility, strength, and overall quality of life.

  • Your impact as a physical therapist (Pt)
  • Consult with patients to assess their physical condition and functional limitations
  • Evaluate and interpret patient assessments and test results
  • Develop individualized treatment plans using a variety of therapy techniques
  • Create personalized rehabilitation programs tailored to each patient’s goals
  • Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing
  • Ensure all treatments are delivered in accordance with the established plan of care
  • Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team
  • Provide clinical support and education to patients, family members, and caregivers
  • Collaborate with interdisciplinary team members to ensure high-quality patient care
  • Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
  • Comply with all applicable rules, regulations, and procedures
  • Attend required meetings as designated by the Director of Rehab
