Consult with patients to assess their physical condition and functional limitations
Evaluate and interpret patient assessments and test results
Develop individualized treatment plans using a variety of therapy techniques
Create personalized rehabilitation programs tailored to each patient’s goals
Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing
Ensure all treatments are delivered in accordance with the established plan of care
Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team
Provide clinical support and education to patients, family members, and caregivers
Collaborate with interdisciplinary team members to ensure high-quality patient care
Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
Comply with all applicable rules, regulations, and procedures
Attend required meetings as designated by the Director of Rehab
