• Clinical Nurse II Part Time– Duke Asthma, Allergy and Airway Center/Pulmonary Job Location: The position is located at 4825 Creekstone Drive, Durham, NC Job Summary: We are seeking a TRIAGE NURSE to be an integral member of our Asthma, Allergy, and Airway Center healthcare team.
• The position will manage this patient population via telehealth in order to coordinate care with providers.
• The Triage Nurse position is based in Durham, NC at a non-clinical office site.
• This position interacts with patients via telephone and secure messaging vs direct patient contact.

Duties: and Responsibilities:

• Provide nursing triage, assessment and education to patients via telephone interactions and secure messaging.
• Manage patient needs including medication management, post procedure instructions and care coordination.
• Serve as a liaison to patients, family members and referring providers with guidance from pulmonary providers.
• Collect and analyze patient data, make recommendations for nursing care plans, evaluate nursing care provided, document and communicate appropriately nursing actions taken and patient responses.
• Delegate tasks and direct the activities of unlicensed care providers (schedulers, Medical Assistants and Staff Assistants).
• Participate in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate assistance or educational offerings.
• Participate in performance improvement and patient experience activities.
• Perform other related duties incidental to the work described herein.
