Salary Range: $125,000 to $135,000 annually
Compensation will be based on, but not limited to, experience, qualifications, credentials and any other relevant information
1. Conduct visits to patients in accordance with local, state and federal regulations. 2. Communicate visit findings to designated members of the interdisciplinary team. 3. Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment). 4. Consult with other members of interdisciplinary team to contribute to the patient's plan of care. 5. Prepare and maintain accurate patient records, charts and documents. 6. Assist in development of clinical practice guidelines/standards in support of quality care as needed. 7. Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Candidates must possess a Master’s degree in Nursing (MSN) or a Doctor of Nursing Practice (DNP) from an accredited program.
