As a Social Services Coordinator, you will play a vital role in supporting the emotional, social, and discharge planning needs of our residents within a skilled nursing setting. You’ll collaborate closely with interdisciplinary teams, residents, and families to ensure individualized care plans are followed, resources are coordinated, and each resident experiences a compassionate, respectful, and supportive environment. Responsibilities

Develop assessments and written plans of care to identify the needs of each resident.
Ensure that all components of the care plan are being followed by reviewing MDS’s and nurses’ notes and attending meetings.
Effectively communicate information concerning a patient’s condition with other departments and government agencies.
