The Clinical Care Coordinator is responsible for conducting assessments, supporting documentation, and developing social care plans aimed at restoring residents to their optimum level. This role also involves supporting residents and families with internal and community resources, particularly concerning Medicare and Medicaid.
Candidates must possess a current LSW or state nurse license and experience in a medical or senior living facility utilizing a team approach to treatment is preferred. Proficiency in Microsoft Office is required, along with strong skills in developing professional relationships and fluency in English.

The Clinical Care Coordinator is responsible for conducting assessments for all per MDS schedule, support documentation and social care plans geared to restoring residents/patients to their optimum level. Supports residents and their families to utilize the resources internally and in the community, particularly concerning Medicare and Medicaid. This position promotes a positive, calm and effective work environment that contributes to teamwork. The Clinical Care Coordinator position requires excellent communication skills with team members, residents and family members as well as a high level of patience and compassion for others.
