The Service Coordinator I provides professional mental health service coordination, including assessment, housing assistance, service planning, and referrals for individuals with serious mental health diagnoses. This role focuses on ensuring stability in relationships, housing, and employment in alignment with recovery principles.
A Bachelor's Degree in a Human Services field like Social Work, Psychology, or Counseling is required, though other fields may be considered with specific coursework. One year of related experience and prior community setting experience are preferred, along with maintaining a safe driving record and specific required clearances/certifications.
Intake and Coordination Coordinate housing and services across multiple life domains Lead and collaborate with treatment/service teams Build relationships with individuals and their support networks Assessment and Service Planning Assess strengths and needs collaboratively Develop and document comprehensive service plans Facilitate interdisciplinary service planning meetings Resource Linkage and Advocacy Identify, link, and coordinate resources Provide culturally competent services Maintain a catalog of community resources Advocate for individuals' needs Training and Documentation Attend training programs for best practices Maintain contact during hospitalizations and transitions Keep accurate records in EPIC Ensure compliance with regulations
