Will cover Baltimore City, County, and surrounding areas. Ideally looking for candidates who can provide a minimum of 100-120 hours per month.

You will visit members within your geographic area of responsibility and conduct an in-home health risk assessment including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam. This evidence-based practice screening will include point-of-care testing (as appropriate), the identification of diagnoses to be used in care management by our Porter Care Guides and active medical management in the furtherance of treatment, and the formulation of a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.
*Required to complete in-home visits in addition to telehealth visits.

\n $125 - $125 an hour Our compensation structure is based on completed visits. We find the per completed visit pay structure to be financially beneficial for our nurses, because it allows for maximizing earnings by seeing more patients within a given time frame. \n
Candidates must have a valid Maryland NP license and at least three years of patient care experience, preferably in primary care or geriatrics. Strong communication skills and a sense of empathy are essential.
