The Physical Therapist will promote and provide physical therapy services in the home to help patients achieve their optimal level of functioning by evaluating and treating deficits related to neurologic, orthopedic, and medical conditions. This includes teaching patients and caregivers to become independent with treatments and chronic illness self-management.

Eddy Visiting Nurse & Rehab Association has an exciting opportunity for an Physical Therapist in a designated area of Columbia County responsible to manage a caseload of patients requiring home-based therapy, primarily post acute or subacute.
Promotes/provides physical therapy services in the home to assist the patient in achieving his/her optimal level of functioning.
Evaluates and treats individuals with functional deficits secondary to neurologic, orthopedic and medical conditions.
Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es)

Work schedule is generally 8-4 during the week. Flexible work schedule.
Weekend rotation: Every 10 weeks and one holiday per year Flexible work schedule.

$43.73 - $56.58 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

Candidates must be a graduate of an approved Physical Therapy program with a Bachelor of Science, Master’s degree, or Clinical Doctorate of physical therapy degree, and hold a current NYS PT license and registration. One year of Physical Therapist experience is required, though home care experience is preferred but not mandatory.
