Summary: The Care Partner is responsible for the overall well-being of each resident through the implementation and facilitation of meaningful, social, emotional, physical and intellectual support and activities. Care Partners are responsible for providing professional, compassionate and quality care for all residents while encouraging them to be as independent as possible. Under certain state guidelines, Care Partners may also provide medication assistance and treatments to residents. The Care Partner is responsible for providing professional, compassionate, and quality care for all residents.
Promptly responds to all resident care needs in line with Avanti Standards. Assist residents while adhering to the care plan in activities of daily living including but not limited to i.e. provide showering and dressing assistance, personal hygiene and grooming assistance, transfer assistance, escort assistance, daily reminders and eating at all times. Motivate and encourage residents to be as independent as possible, while still providing any needed assistance. Observe residents’ conditions, measuring and recording food and liquid intake and output and vital signs within regulations, and report changes to Charge Nurse. Assist and support residents in the participation of activities while providing motivation and encouragement. Remind residents of different activities offered daily while providing alternate options such as fitness, art classes, music stimulation and etc. Maintain a clean, safe and orderly environment for the resident. Providing general housekeeping following cleaning schedules for resident laundry, bedrooms, dining area, living space, bathrooms, kitchenette, etc. Attend mandatory monthly in-service and safety meetings. Promptly responds to all resident care needs. Other duties and special projects as assigned by Management Dining Responsibilities Deliver and retrieve meal trays to rooms as needed. Clean and reset dining rooms as needed. Serve meals to residents in dining room or suites. Also, assist in preparing meals following pre-planned menus when needed. Encourages residents to eat a proper diet and take adequate fluids while respecting the resident’s right to free choice. Also, report changes in residents’ eating habits to the supervisor. Comply with all food handling and safety guidelines including: maintaining required certifications; wearing hairnets when in the kitchen and dining area; and washing hands thoroughly before handling food.
