Nurse Practitioners provide longitudinal care as integral members of Care Teams, performing annual health assessments and completing post-discharge visits for an intimate panel of patients. Primary responsibilities include evaluating and treating center patients, coordinating care with other providers, and performing comprehensive in-home assessments.
Our Care Teams include a Physician, Nurse Practitioner, Care Coordinator, Medical Assistant, and support from a Nurse Care Manager. Nurse Practitioners are integral members of our Care Teams as they provide longitudinal care and build meaningful patient relationships; this creates trust and helps our patients understand and feel connected to their care plan.
Together this team is responsible for providing and coordinating care for an intimate panel of patients in our neighborhoods. As a key member of Sage Health care team, you will perform annual health assessments, complete post discharge visits and deliver cost-effective, quality care to assigned patients both in our centers and patients' homes.
Candidates must possess a current and unrestricted NP or PA license and have at least two years of experience practicing as an NP or PA, preferably in Primary Care or Adult/Geriatrics. Understanding of HCC documentation, ICD-10 Coding, and HRAs is required, along with a DEA certificate and BLS certification.
