The Pharmacy Technician will assist in collecting Home Medication Histories for admitted patients by utilizing provided resources to identify recent outpatient dispenses, interviewing patients, and contacting pharmacies. This role requires accurately documenting all collected information, primarily within the Emergency Department but potentially on inpatient floors, always working under the direct supervision of a registered pharmacist.
Candidates must possess a high school diploma or GED equivalent and hold a current Pharmacy Technician License in the State of Illinois. Additionally, they must either have current Pharmacy Technician Certification or obtain CPhT within 90 days, or have successfully passed the PTCB or ExCPT exam pending certification, or hold a current Illinois 'Student' designation license.
