The Licensed Practical Nurse will provide skilled nursing care ordered by the physician according to the plan of care, which includes assessing patients and collaborating with the supervising RN and other interdisciplinary group members. Responsibilities also involve educating patients and families, managing pain and symptoms, and accurately documenting all visits in the Electronic Health Record.
Candidates must be a graduate of an accredited nursing program and hold current licensure as an LPN or LVN in the state of practice, along with valid driver's documentation and CPR certification. Preferred qualifications include a Master's degree in ministry, counseling, or social work, CPE training, and specific experience in acute care or home care settings.
