About the role

The clinician is responsible for developing, implementing, and evaluating exercise and education programs for clients in Cardiac and Pulmonary Rehabilitation Phases I, II, and III. This includes collaborating with the healthcare team, documenting patient progress, and counseling patients and families on disease management and lifestyle modifications.

Overview Under direction to perform work involving the development, implementation and evaluation of the exercise and education experiences of clients referred to Phases I, II, and III Cardiac Rehabilitation and Phases II and III Pulmonary Rehabilitation. Participates in meeting education of Heart Failure patients and follow up of their progress. Participates with community events related to the CV Service Line.

Responsibilities

PRINCIPAL DUTIES AND RESPONSIBILITIES(*Essential Functions) Collaborates with the Exercise Specialist to inform patients of need to schedule annual stress tests, to complete body composition measurements and a lipid profile (at M.D. discretion), to prepare client progress reports, and to schedule and complete follow-up outcomes data collection sessions. Coordinates staff responsibilities during emergency situations. Participates in a review of emergency procedures on a monthly basis. Coordinates the client's care by collaborating on an ongoing basis with all members of the health care team, including but not limited to physician, staff nurses, dietitians, respiratory therapists, social workers and Cardiopulmonary Rehab staff. Documents information in the client's chart in accordance with hospital and department policy. Counsels/educates client's and their families regarding their disease process therapy and appropriate lifestyle modification. Counseling/education sessions may be done in a group or individual session. Teaches (Phase II and III) group session on an assigned basis and (Phase I, II and III) individual sessions as needed. Develops/revises education outlines as assigned. Demonstrates Phase I, II, and III exercises. Supervises and evaluates client's heart rate, rhythm, blood pressure and symptomatic responses to prescribed exercises. Communicates observations to other staff members on an ongoing basis. Develops at the time of referral (Phase I) or orientation (Phases II and III) an exercise and education care plan for each client. Reviews the care plan each time the client is seen and updates as needed. Communicates the care plan to other staff members both verbally and in writing on an ongoing basis. Evaluates program effectiveness through staff interaction, patient surveys and needs assessment on an ongoing basis. Make recommendations to the Team Leader of Cardiopulmonary Rehabilitation regarding program development. Participates in new program development and implementation. Interviews clients at the time of referral (Phase I) or orientation (Phases II or III) and completes the appropriate forms and formulates a nursing assessment. Communicates this assessment to other staff members on an ongoing basis. Provides the Team Leader of Cardiopulmonary Rehab with proper billing information on a daily basis in accordance with established departmental procedures.

Qualifications

JOB SPECIFICATIONS(Minimum Requirements) KNOWLEDGE, SKILLS, AND ABILITIES current certification as a CPR instructor desirable. Knowledgeable of cardiovascular and pulmonary anatomy, physiology and path physiology. Successful completion of a basic arrhythmia interpretation course. Experience in adult education desirable. Demonstrates proficiency in the cardiovascular and pulmonary assessments. EDUCATION Associates Nursing Required Bachelors Nursing Preferred Six months of which are in ICU, CCU, or IMCU areas, caring for clients with Coronary Artery Disease or equivalent combination of education and relevant work experience. 5 Required 1 Healthcare/Medical - Critical Care Medicine Required Licensed Registered Nurse Certified Advance Cardiac Life STANDARDS OF BEHAVIOR Meets the Standards of Behavior as outlined in Personnel Policy and Procedure #1, Employee Relations Code. CONFIDENTIALITY/HIPAA/CORPORATE COMPLIANCE Demonstrates knowledge of procedures for protecting and maintaining security, confidentiality and integrity of employee, patient, family, organizational and other medical information. Understands and supports the commitment of Methodist Hospitals in adhering to federal, state and local laws, rules and regulations governing ethical business practices for healthcare providers. Requirements

Candidates must be a Licensed Registered Nurse with an Associate's degree required and a Bachelor's preferred, possessing knowledge of cardiovascular/pulmonary anatomy and physiology, and successful completion of a basic arrhythmia interpretation course. A minimum of six months of experience in ICU, CCU, or IMCU caring for Coronary Artery Disease patients is required.
