The LPN will provide skilled nursing care in patient homes according to the RN's plan of care, performing clinical tasks like medication administration and wound care. Responsibilities also include observing, documenting, and reporting patient status changes, as well as educating patients and caregivers on disease management.
Candidates must be graduates of an accredited practical nursing program and hold a current LPN license in Maine along with CPR certification. A minimum of 2–3 years of recent nursing experience in acute care, rehabilitation, or home health is required, along with reliable transportation.

Perform clinical tasks including medication administration, wound care, and monitoring of patient conditions. Observe, document, and report changes in patient status to the supervising RN and care team. Educate patients and caregivers on disease management, medications, and preventive care to support recovery and independence. Promote patient and family engagement in care to improve health outcomes and self-management. Assess the home environment for safety and make recommendations to reduce risks and support recovery. Collaborate with an interdisciplinary team to support patient goals and continuity of care. Participate in health education and preventive care initiatives within the community. What You’ll Bring: Graduate of an accredited practical nursing program; successful completion of an approved pharmacology course. Current LPN license in the State of Maine and CPR certification required. Minimum of 2–3 years of recent nursing experience in acute care, rehabilitation, or home health setting. Strong clinical assessment skills with the ability to work independently in a home setting. Experience with wound care, medication management, and chronic disease education preferred. Excellent communication and documentation skills, with the ability to collaborate effectively with an interdisciplinary care team.
