The LPN will provide skilled nursing care to patients in their homes, including medication administration, wound care, and monitoring patient conditions according to the RN's plan of care. Responsibilities also involve educating patients and caregivers, documenting changes in status, and assessing home safety.
Candidates must be graduates of an accredited practical nursing program and hold a current LPN license in Maine along with CPR certification. A minimum of 2 to 3 years of recent nursing experience in acute care, rehabilitation, or home health is required, along with reliable transportation.

Perform clinical tasks including medication administration, wound care, and monitoring of patient conditions. Observe, document, and report changes in patient status to the supervising RN and care team. Educate patients and caregivers on disease management, medications, and preventive care to support recovery and independence. Promote patient and family engagement in care to improve health outcomes and self-management. Assess the home environment for safety and make recommendations to reduce risks and support recovery. Collaborate with an interdisciplinary team to support patient goals and continuity of care. Participate in health education and preventive care initiatives within the community. What You’ll Bring: Graduate of an accredited practical nursing program; successful completion of an approved pharmacology course. Current LPN license in the State of Maine and CPR certification required. Minimum of 2–3 years of recent nursing experience in acute care, rehabilitation, or home health setting. Strong clinical assessment skills with the ability to work independently in a home setting. Experience with wound care, medication management, and chronic disease education preferred. Excellent communication and documentation skills, with the ability to collaborate effectively with an interdisciplinary care team.
