The Hospice RN acts as the primary contact, delivering and documenting skilled nursing care based on the individualized Plan of Care in various patient settings. Responsibilities also include collaborating with an interdisciplinary team, providing emotional support, and educating caregivers on patient needs.
Candidates must possess an active RN license and at least one year of experience in hospice, home health, ICU, oncology, geriatrics, or related fields, along with reliable transportation and CPR certification. A compassionate, patient-centered approach is essential for guiding patients and families through end-of-life care.
