About the role

The Advanced Practice Nurse will conduct patient visits according to regulations, communicate findings to the interdisciplinary team, and facilitate access to appropriate support models of care. Responsibilities also include consulting on the patient's plan of care, maintaining accurate records, and ensuring collaboration among patients, caregivers, and medical directors.

Now Hiring - Advanced Practice Nurse - Bridge Care Bergen County NJ Salary Range $125,000 to $135,000 a year Advanced Practice Nurse will be responsible for, but are not limited to: Conduct visits to patients in accordance with local, state and federal regulations.

Communicate visit findings to designated members of the interdisciplinary team.

Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).

Consult with other members of interdisciplinary team to contribute to the patient's plan of care.

Prepare and maintain accurate patient records, charts and documents.

Assist in development of clinical practice guidelines/standards in support of quality care as needed.

Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Education

Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.

Professional

Experience: Must have a Nurse Practitioner's license in the state of operations Must have 1+ years' experience working within Family Practice, General Practice, Geriatric care or related field, Must have a valid driver's license and a reliable source of transportation Must be willing to travel Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required. Requirements
