Full-Time Homecare Physical Therapist - Capital Region $5,000 SIGN ON BONUS Eddy Visiting Nurse & Rehab Association has an exciting opportunity for an Physical Therapist in a designated area of our Capital District responsible to manage a caseload of patients requiring home-based therapy, primarily post acute or subacute. Can choose either a 37.5 hour work week or 40 hour work week

What you will do: Promotes/provides physical therapy services in the home to assist the patient in achieving his/her optimal level of functioning. Evaluates and treats individuals with functional deficits secondary to neurologic, orthopedic and medical conditions. Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es) Experience the rewards of appreciation from being able to address the whole patient Work schedule is generally 8-4 during the week. Flexible work schedule.
Weekend rotation:
Every 6 weeks and one holiday per year

Range: $78,208 - $100,048 for 40 hour work week or $73, 320 - $93,795 for 37.5 hour work week. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
