The Nurse Practitioner will conduct comprehensive assessments and manage symptoms for patients with serious, chronic, or life-limiting illnesses, focusing on improving quality of life. Key duties include facilitating goals-of-care discussions, coordinating care with interdisciplinary teams, and educating patients and families.
Candidates must possess a Master’s or Doctoral degree in Nursing and hold current NP licensure in Oregon, along with Nurse Practitioner certification in a relevant specialty. Experience or training in palliative care or hospice is preferred, alongside strong clinical judgment and communication abilities.
Job Title: Palliative Care Nurse Practitioner (NP)
We are seeking a compassionate Palliative Care Nurse Practitioner to provide patient-centered care for individuals with serious, chronic, or life-limiting illnesses. The NP will manage symptoms, support complex decision-making, and coordinate care in collaboration with interdisciplinary teams to improve quality of life for patients and their families. Key Responsibilities: Conduct comprehensive assessments and manage patients with complex chronic or life-limiting illnesses. Provide symptom management for pain, dyspnea, nausea, fatigue, anxiety, and other distressing symptoms. Facilitate goals-of-care discussions and advance care planning, including end-of-life decisions. Collaborate with physicians, nurses, social workers, and other team members to coordinate patient care. Educate patients and families about disease processes, treatment options, and self-management strategies. Document patient care accurately in accordance with regulatory and institutional standards. Participate in quality improvement initiatives, program development, and continuing education.
Master’s or Doctoral degree in Nursing from an accredited program. Current NP licensure in the state of practice. Certification as a Nurse Practitioner (family, adult-gerontology, or relevant specialty). Experience or training in palliative care or hospice preferred. Strong clinical judgment, communication skills, and ability to manage complex patient care. Work Environment: Outpatient clinic, hospital, or home-based palliative care settings. Collaborative, interdisciplinary team environment with opportunities for professional growth and continuing education. Must maintain state certification and/or licensure.
