The Physical Therapist will evaluate patients' functional limitations and develop individualized treatment plans to restore mobility and alleviate pain in a home health setting. They are also responsible for collaborating with an interdisciplinary team and documenting patient progress using digital platforms.
Candidates must hold an active Physical Therapist license in Texas and a graduate degree or higher in Physical Therapy from an accredited program. Experience in home health or rehabilitative settings is preferred, along with strong communication skills.
