Community Health Nurse: Tapestry 360 Health
A leading member of the health center care team, the Tapestry 360 Health (Tap360) Community Health Nurse assists with direction of daily patient visits, providing assessment and triage, care coordination, and health education and information to patients and families in a culturally appropriate and relevant manner. The nurse works with the health center team to assure the maintenance of a safe patient care environment and safe transitions in care both within Tap360 and with other community agencies and contributes to quality assurance and other organizational activities as needed.
Tapestry 360 Health offers a comprehensive benefits package including health insurance, dental insurance, retirement savings plans, paid time off, continuing education support and eligibility for the Public Service Loan Forgiveness (PSLF) program. Pay Transparency: The salary range for this position is $70,000 – $90,000 annually. Actual compensation will be determined based on factors including, but not limited to, relevant experience, qualifications, internal equity, and organizational needs.
